Enter your data online

If you have any completed NBMP survey forms you have yet to return to us, you may prefer to submit your results to us using the data entry forms on the right.

The fields in the online data entry forms are in the same order as the fields on your printed survey forms. The best way to move from one field to the next is to use the Tab key on your computer keyboard.

Before clicking "Submit" please double check that you have inputted all the information correctly.

If you would like to also send us a copy of your original printed form then that will enable us to do some spot checks to ensure that this new online data entry system is working correctly.
